Outpatient Services
Community Bridges is dedicated to providing outreach and education to the community at large. Our services reach the mainstream public through our education efforts and our clinical services provide quality treatment for substance use disorders for men and women. We believe addiction is a disease that carries tremendous consequences for individuals, families, the workplace, and the community as a whole and that prevention is key to combating substance abuse.
Comprehensive Outpatient Programs Statewide
Community Bridges’ numerous Outpatient programs provide co-occurring capable and enhanced services to accommodate the individual, and help them and their families acquire the skills they need to reclaim their lives from the grips of addiction. Our Outpatient programs, located throughout Maricopa and Gila counties, are best practice behavioral and cognitive modalities. Outpatient programs provided by Community Bridges make every reasonable effort to actively engage family members throughout the treatment process. Coordinating care with either existing professional services involved in the individual’s life or identifying those resources necessary to assist in creating successful outcomes is one of our primary goals. Each of our programs provides customized treatment plans based upon individual’s unique level of need. Community Bridges offers the following Outpatient Services:
Criminal Justice Based Outpatient Services Programs
Community Bridges provides services to the Courts and Criminal Justice System throughout Maricopa, Pinal, and Gila counties. Medical Detoxification for individuals experiencing withdrawal from Alcohol, Opiates, or Benzodiazepines is available as an emergency life saving intervention which can then be followed by immediate entry into our or providing state of the art outpatient treatment services. Community Bridges has the Criminal Justice client and professional in mind when developing our programs and reporting procedures.
The following crimeogenic factors are assessed at the time of initial assessment and throughout the entire treatment process:
- Current & past substance history
- Employment & Education
- Marital/Family Relations
- Social Peer Network
- Antisocial Cognitions (Attitudes/Orientation/Thinking)
- Community Functioning
- Mental Health Status
Each of these key areas are continually addressed in a group setting but also in individualized counseling sessions with parties currently involved in the client’s treatment plan.
Drug Court
Community Bridges is currently the sole contracted provider of substance abuse treatment services to the Maricopa County Drug Court. We offer a 52-week model that works closely with both the court and assigned probation officer to ensure the client is at the appropriate level of care throughout the course of treatment. The cognitive behavioral model used is specific to address both addiction and criminogenic factors:
- Standard Outpatient Services: minimum of 80 hours of treatment over a 20 week period focusing on core issues of addiction, criminality, co-occurring disorders, and other psychosocial stressors that lead to relapse.
- Relapse Prevention: minimum of 15 group hours over a 10 week period where the client learns focused skills to help her/him deal with real life triggers and psychosocial issues.
- Aftercare: minimum of 1 hour of treatment every other week for 22 weeks to check in and review the clients success in implementing what has been learned in treatment as well as continue to receive support for any ancillary issues that arise as part of the ongoing recovery process.
In addition to this comprehensive level of support, Community Bridges has designated staff members who function as Court Liaisons. Our Court Liaisons attend weekly court sessions, staffings, and submit comprehensive monthly reports to both the probation department and the presiding judge upon his or her request. This additional support helps ensure that there is a clear line of communication and coordination of care between criminal justice and Community Bridges treatment professionals.

Drug Treatment Education Fund (DTEF)
Community Bridges is a contracted DTEF provider in Maricopa County. Our DTEF program offers both Intensive (40 sessions) and Standard (30 sessions) Outpatient treatment services. Community Bridges utilizes the Los Angeles based Matrix Model to provide treatment services that focus on the following areas:
- Early Recovery Skills
- Relapse Prevention
- Family Education & Treatment
- Development of Social Supports
As with our other programs, the assigned probation officer or referring court is included in all communication and have input into treatment team appraisals, treatment decisions, and regular progress reviews.
Pre Trial Services (PSA)
As with the Drug Court and DTEF, Community Bridges is contracted to provide both treatment and education services as part of the Pre Trial Services program. Each individual referred receives a comprehensive assessment as well as the Adult Substance Use Survey (ASUS) to determine the most appropriate level of care for the individual. This level of care can extend from basic substance abuse education to primary treatment.
Substance Abuse Education (SAE)
Community Bridges provides and intensive Substance Abuse Education (SAE) program for those individuals that may not be appropriate for primary treatment, as part of a diversion program or for the individual that has received a DUI/DWI. These education classes are for a minimum of 16 hours and are provided in either two (2) eight (8) hour sessions or in eight (8) two (2) hour sessions. The SAE program is very intense that is geared to assist the individual in recognizing the full consequences of their actions and alternatives to substance use. These are a few of the areas in the 16 hours of education:
- Physiological and psychological effects and aspects of substance use;
- Effects and consequences of operating a motor vehicle;
- Stages of abuse/addiction;
- Co-Occurring Disorders;
- Self-Assessment & Screening;
- Family Education;
- Alternatives to substance use;
- Criminal penalties of substance use;
- Resources available in the community for ongoing support.
Participants will receive a pre and post test for each educational section. A passing score is required in order to obtain a certificate of completion.
Probation & Court Reports
Community Bridges believes that the key to successful treatment is the participation of all parties currently involved in the client’s case. This of course includes both the referring Court as well as the assigned Probation Officer. Communities Bridges provides both the referring Court and assigned probation officer monthly progress reports that cover:
- Attendance
- Significant Events (hospitalization, loss of job, dirty U/A, etc.)
- Functional Improvement
- Symptomatic Improvement
- Results of any pre/post tests
- Treatment completion date & number of hours completed
- Summary of overall progress
Community Bridges will also notify the referring party within 24 hours if the client does not attend a scheduled treatment appointment. Copies of discharge and treatment plans are also provided.
Primary Outpatient Treatment Services
If the client does not fit into one of the above programs, Community Bridges also offers both Intensive and Standard Outpatient programs which provide co-occurring capable treatment for a minimum of 12 to 16 weeks. These programs are utilized as a step down from a higher level of care; or utilized for the individual who has not accessed residential treatment services but requires a higher intensity of services; or for the individual who has received a DUI/DWI. Both family and individual care are also available based upon the client’s presenting need and request.
Community Bridges utilizes the American Society of Addiction Medicine (ASAM) Patient Placement Criteria to determine the intensity of services at the time of admission. As with our other programs the treatment models utilized are evidenced based and cognitive behavioral in nature.
